AIRLINE INFORMATION REQUIRED:
You must submit your Arrival Date, Airline Name, Flight # and time of arrival to Panama City.
Departure Date, Airline Name, Flight # and time of Departure from Panama City.
You also must provide us a copy of your itinerary.
We also need the reservation # on your itinerary. This is very important in order for us to confirm your flights. (To locate # look at the itinerary, find one of the following: Confirmation #, Ticket #, Record Locator # or Reservation #) Without this number we cannot reconfirm your flights or make changes to your itinerary if you request us to do so, while in Piñas.
HOTELS: You may choose the hotel of your preference, but the ground transfers included are only for the Sheraton, our host hotel, or the Bristol Hotel.
It is best that we make your hotel reservation in order to coordinate your transportation.
A VISA, MasterCard or American Express number is required to hold the reservation.
This form must be in the Orlando Office at least 60 days prior to arrival in Panama City.
